At the time of booking an appointment with the Psychiatrist please provide the following details:

Date of Birth
Mailing Address
GP details (Name, Address and Telephone number of your GP) 
Insurance Details (if Applicable)

Meeting the Consultant Psychiatrist

You are welcome to bring along a relative, friend or carer. Please bring the list of your current medications with you on the day of consultation.

Cancellation Policy

Please note that appointments cancelled by the patients/clients without more than 48 hours prior notice will be charged at a flat rate of £50 for missed appointments. However, if the appointments are cancelled on the date of appointment, there's a charge of £100.
